- Foster parents will be notified by the supervising agency at least 60 days in advance of their annual licensure date. An application may be enclosed with this notice or the supervising agency may contact the family to discuss whether they wish to continue to be licensed to provide foster care. These contacts must be documented in writing.
- Step One of the application must be completed by the foster parents to begin the process of the annual licensing study.
- The evaluation of the home and family will be completed by the social worker on the Family Foster Home Licensing Report (SFN 1941) and submitted with the appropriate recommendations to the Director of the Regional Human Service Center or their designee.
- The licensing study and materials must be submitted to the Regional Human Service Center 15 days prior to the expiration date of the current license and acted upon with that 15 day period of time.
